During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 3/23/26 from 10:10 am to 11:45 am. The inspector reviewed compliance in the areas listed above. There were 32 children present and 10 staff. The inspector reviewed 3 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards. Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center must obtain the results of a central registry search (CRS) for an employee within 30 days of employment.
Staff # 2's CRS results were not obtained by the center to this date, and were unavailable for review.
Plan of Correction: Submit new CRS request.
Standard 8VAC20-780-240-I
Documentation of orientation training shall be kept by the center in a manner that allows for identification by individual staff member, is considered part of the staff member's record.
Staff # 2's record did not have documentation of orientation training.
Plan of Correction: Staff #2 completed Orientation and it is now indicated in their file.

Materials and equipment available shall be age and stage appropriate for the children.
Children in the 2 year old classroom were observed playing with kinetic sand that was intended for children 3 years old and up.
Plan of Correction: Discussion with 2 year old classroom teachers and kinetic sand was removed immediately. Searching for more page appropriate secondary options.

Staff must ensure the handwashing of children after toileting or exposure to bodily fluids.
One infant in the infant classroom was observed getting a diaper change. The infant's hands were not washed after diapering, and the infant was placed in the crib.
Plan of Correction: Employees of said classroom were spoken to and are under review for proper diaper changing procedure.

Staff records must contain complete information.
Staff # 1's record was missing documentation of one written reference check.
Plan of Correction: Discuss with staff #1 and received a new reference. Followed up with the reference and everything checked out.
