Inspector Notes

An unannounced monitoring inspection was conducted on Monday, November 13, 2023. The inspection began at approximately 10:50am and concluded at 2:00pm. There were 109 children present, with 22 staff supervising. Children were observed playing outside, playing with toys, eating lunch, and preparing for nap. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 10 child records and 10 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

(Repeat violations) Based on observation, areas of the center, inside, were not maintained in a clean, safe, and operable condition.

Evidence:
1. In the toddler classroom, there was chipped blue paint on the boarder of the window seal.
2. In preschool classroom, there was chipped green paint on a boarder on the wall in the front of the classroom under the bulletin board.
Plan of Correction: Repair chipped paint.

Based upon staff interview, the facility has not ensured that a current written list of all children's allergies, sensitivities, and dietary restrictions is maintained and documented in the allergy plan that is dated and kept confidential in each room or area where children are present.

Evidence:
1. The dietary restrictions lists in the classrooms were posted in a way that the information was not covered or protected to keep it confidential.
Plan of Correction: Cover all food restrictions.

Based on observation, the center failed to ensure that children?s hands are washed after toileting and staff?s hands are washed after a diaper change.
Evidence:
1. In the toddler classroom, Staff #9 was observed changing a diaper and did not wash their hands or the child?s hands after the diaper change.
Plan of Correction: Retrain all staff on proper diapering procedures.
