Inspector Notes
An unannounced monitoring inspection was conducted on May 24, 2024 from 8:40 AM until approximately 12:05 PM. There were 120 children present, ranging in ages from 3 months to 5 years, with 24 staff supervising. The inspector observed children participating in circle time activities, playing with toys, eating breakfast, and interacting with staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A sampling of medications was reviewed for 5 children. A total of 12_ child records and 12 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations
2Evidence: The poured in place surfacing on the Pre-K playground, 3-year and 4-year-old playground, and the infant and toddler playground, had multiple circular areas approximately 4-5 inches inches wide where the poured in place surfacing was missing.
Evidence:
Evidence: In the PreK 1 classroom, there was a can of air freshener spray and a container of hand sanitizer in an unlocked cabinet in the restroom that was accessible to children. The can of air freshener and the container of hand sanitizer were labeled "keep out of reach of children".
