During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced monitoring inspection was initiated and completed on March 26, 2026, in response to a self-report received by the licensing office on March 17, 2026, relating to forbidden action and behavior guidance. The inspector reviewed 1 child?s record and 1 staff record on-site. This inspection included interviews, document review, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on April 14, 2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-400-B
Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

On March 13, 2026, Staff #1 forcefully lifted child #1 age 2, by the left arm off a step stool while child #1 was playing at the sink with another child. Staff #1 lifted child #1 approximately 1 ? feet off the ground before child #1 landed on both feet. Child #1 was observed crying.
Plan of Correction: Retrained entire staff on behavior guidance and positive guidance.
Standard 8VAC20-780-410-1
The following actions or threats thereof are forbidden: roughly handling a child.

Staff #1 roughly handled Child #1, age 2, when they forcefully lifted child #1 by the left arm off a step stool. Child #1 was observed crying.
Plan of Correction: Staff #1 was terminated immediately after incident (3/12/26). Entire staff retrained on redirection and forbidden action.
