Inspector Notes
A monitoring inspection was conducted on 01/08/2024 from approximately 10:41 a.m. to 6:00 p.m. There were 148 children present ranging from 4 months old to 5 years old and 23 staff supervising. The inspector reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. Ten child (10) records ten (10) staff records were reviewed. The information gathered during the inspection determined violations with applicable standards or law. Please sign and complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Staff record for S7 documented on the sworn statement having lived out-of-state in the last five years. The record for S7, employed 07/28/2023 did not contain a sex offender searches for two states in which the individual lived in the preceding five years, the results of the out-of-state child abuse and neglect searches, and a criminal history name check for one of the two states the individual lived in the preceding five years.
Staff record for S8 documented on the sworn statement having lived out-of-state in the last five years. The record for S8, employed 06/21/2023 did not contain the results of the out-of-state criminal history name check and sex offender registry by the date of hire. The checks were dated 07/11/2023.
Based on staff record review, the provider failed to ensure the record contained central registry findings within 30 days of employment.

Licensing inspector observed in the Tugboat Room a container of liquid hand soap accessible to the children at the classroom sink with a warning label of keep out of reach of children and pets.

The record for Child C4, enrolled 12/1/2022, parent confirmation that the required information in the child?s record is up-to-date. This is an annual requirement.
Based on record review, the provider did not keep the required records for each staff person.
