An unannounced, on-site monitoring inspection was completed on 01/28/2025. The on-site inspection began at 10:35 a.m. and ended at 4:00 p.m. The inspector reviewed compliance in the areas listed above. There were 160 children present and 19 staff. The inspector reviewed 10 children?s records and 10 staff records. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 02/28/2025. A POC submitted after this date will not appear on the public website.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. S2, employed for more than 3 months, did not have documentation of requesting a criminal history check from a different state or a check of the sexual offender registry. S7, employed less than 30 days, did not have documentation of requesting the criminal history check from a one state or a check of the sexual offender registry for two states.

The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. C4, who had been attending the center for one day, did not have documentation of an immunization record until the second day of attendance.

If a child six months of age and younger has had a physical examination prior attendance, the physical exam should be within two months prior to the date of attendance. The physical examination for C5 was not within 2 months prior to the attendance.

Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. The TB screening for S7 was more than 30 days of the date of employment.

Prepared infant bottles shall be refrigerated, dated and labeled with the child?s name. Two bottles for one infant did not have the infant?s name, two bottles for a second infant were not dated, and a food container for a third infant was not dated.
