Inspector Notes
An unannounced monitoring inspection was conducted 11/16/2023 from 10:00am-1:35pm. There were 31 children directly supervised by 7 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, menus, 3 staff records, 6 children?s records, 6 medications, emergency drills, injury reports, emergency supplies, and policies were inspected. Children were observed practicing counting, learning the alphabet and letter sounds, participating in circle time, reading the story "How to Catch a Turkey", and eating snack which consisted of graham crackers and apple sauce. There was an appropriate supply of age appropriate materials for the children. There was an adequate number of CPR/First Aid, Daily Health Observation, and MAT trained staff on site at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Violations
10Evidence: Staff #1 (Date of Hire: 7/20/2023) has resided in Florida within the last 5 years and did not have documentation of a child abuse and neglect central registry search on file.
Evidence: Staff #3 (Date of Hire: 10/5/2023) did not have a Sworn Disclosure Statement on file at the time of inspection.
Evidence: There were two holes in the drywall in the School Age classroom.
Evidence:
1. In the 2's classroom, there was a disinfectant spray and a bleach water spray bottle stored in an unlocked cabinet. There was also an odor eliminator spray with the warning "Caution - Keep out of reach of children." that was sitting on the counter in the bathroom.
2. In the Toddler's classroom, there was a bleach water spray bottle sitting on the changing table.
Evidence: In the 2's room, a disinfectant spray and bleach water spray were stored in a cabinet above plates, cups, and spoons.
Evidence: In the 3's classroom, there were 5 outlets uncovered. In the School Age classroom, which was occupied by the 3's and 4's classroom during the inspection, had 12 outlets uncovered.
Evidence: Child #2 (Age 5 months) has not been provided with a crib to sleep in the Infant room. Child #2 was observed sleeping in a swing at the time of inspection. Staff advised the infant does not sleep in a crib because the parent hasn't brought in a fitted sheet yet so the infant sleeps in a bouncer or the swing.
Evidence: In the Infant room, Child #1 (13 months) had a nap mat with a pillow and attached filled comforter in the assigned crib. Staff stated that the child uses the pillow and filled comforter in the crib during naps.
Evidence: In the Toddler room, there was a Lunchable observed in the refrigerator that was not labeled or dated.
Evidence: Child #6's record was missing two phone numbers for emergency contacts. Child #5's record could not be located at the time of inspection.
