An unannounced monitoring inspection was conducted 2/21/2024 from 10:05am through 1:15pm to investigate a self-reported incident received by the Northern Licensing Office regarding staff engaging in a forbidden action. Staff and child records were reviewed, staff interviews were conducted and the center's video recording was reviewed. A violation was found regarding a forbidden action and is identified in the Violation Notice.

Based on review of the center's own video recording, a staff member engaged in a forbidden action of striking a child.
Evidence: On 2/7/2024 at approximately 9:34am Staff #1 quickly approached Child #1 after the child threw a book down, bent down in the child's face with an angry expression, and jabbed Child #1 in the left side rib area.
