Inspector Notes

An unannounced monitoring inspection was conducted 4/3/2024 from 11:35am through 12:15pm to investigate a self-reported incident received by the Northern Licensing Office regarding hazardous substances. Observed evidence was reviewed, staff interviews were conducted, and staff records and documentation was reviewed. Violations were found relating to the self-reported incident and are identified in the Violation Notice.

Based on observed evidence, purses containing medications were stored accessible to children.

Evidence: In the Twos room on 3/26/2024 at approximately 1:29pm, Staff #1 left a red purse containing prescription medication on the children's blue table while the staff member went into the bathroom for approximately two minutes. There were children napping in the room while the red purse was left unattended.

Based on observed evidence, the center did not ensure that smoking was prohibited in the interior of the center.

Evidence: In the Twos room on 3/26/2024 between approximately 1:29pm and 1:33pm, Staff #1 used an electronic smoking device several times while children were napping in the room.
