During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 10/29/2024. The on-site inspection began at 10:25am and ended at 12:50pm. The inspector reviewed compliance in the areas listed above. There were 36 children present and 8 staff. The inspector reviewed 5 children?s records and 4 staff records on-site on 10/29/2024. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 1/20/2025. A POC submitted after this date will not appear on the public website.
Violations
5Child #1 is under the age of two years and their updated immunizations were due seven months ago.
Child #1 has been attending the center for over nine months and did not have a physical examination on file.
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition.
The gate post has detached from the fence surrounding the toddler playground which is located near the exterior door children use to travel to and from the main playground. The gate post and fence had one nail with the sharp end protruding approximately 1.5" and another nail with the blunt end protruding approximately 1.5".
When food is brought from home it shall be clearly dated and labeled in a way that identifies the owner.
Several items of food in the refrigerator in the Infant room were not dated.
A nine month old infant was observed drinking a bottle from a rocking swing.
