When staff are supervising children, they shall always ensure their care, protection, and guidance.
Video footage from January 13, 2026, shows Child #1 (age: 16 months) playing on the indoor play equipment. Child #1 attempts to walk down the equipment steps, loses balance, and falls down the equipment steps, hitting their face on the ground and leaving an abrasion on the left cheekbone, underneath the eye . Video footage shows Staff A and Staff B on-and-off their cell phones multiple times, with neither staff supervising the children on the play equipment.

Plan of Correction: Retrained staff on cell phone policy and require staff to lock cell phones while supervising children.

During the center's hours of operation, one adult on the premises shall be in charge of the administration of the center.
On January 13, 2025, Staff #2 reported that following a child injury, at approximately 3:30pm, they went looking for management, however, there was no one in charge of the administration of the center available to assist. Staff #3 confirmed that both administrators were out of the building at that time.

On January 13, 2025, Staff #1 and Staff #2 were supervising the Infant (ages: 6 weeks-16 months) classroom. Both staff members are Assistant Teachers and do not meet the qualifications of a program leader or director.
