During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 5/23/25 on, as a part of licensure period. The on-site inspection began at 9:10am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were 12 children present and 3 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/19/2025. A POC submitted after this date will not appear on the public website.

The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. The most recent fire inspection report is dated 11/1/2023.

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in safe condition. In the infant room there is a dangling cord within reach of children from a laptop to the outlet. The dangling cord poses a toppling hazard.

Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. There are multiple cleaning chemicals stored within reach of children in an unlocked closet between the infant room and the kitchen.

When children are in ongoing mixed age groups, the staff-to-children ratio and group size applicable to the youngest child in the group shall apply to the entire group. Upon arrival there were 11 children present in one with two staff. The ages of the children present ranged from 14months to 5years. Therefore, an additional staff was needed.

The following nonmedical emergency supplies are required one working, battery-operated radio in each building used by children. The battery operated radio was not working during the inspection.

Documentation of child updates and confirmation of up-to-date information in the child's record. Three out of five records reviewed did not have documentation of information update after 2023.

Standard 8VAC20-780-80-A
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. There is not an individual attendance record for each group of children. There is one attendance sheet for the entire building located at the entrance.
