Inspector Notes

An unannounced inspection was initiated on 08/04/2025 and completed on 08/15/2025 in response to a complaint received by the licensing office on 08/04/2025 regarding allegations related to the Code of Virginia. The inspector reviewed 1 child?s record and 5 staff records on-site and electronically. This inspection included document review, tour of the facility, interviews and observations.

The preponderance of evidence gathered during the inspection supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/22/2025. A POC submitted after this date will not appear on the public website.

The required ratio for children ages 36 months to five years is one staff member per 10 children.

Staff 3 was the only staff member supervising 15 children in Classroom 3. The youngest child in the group was 4 years old; this group of children required 2 staff.
Plan of Correction: Already corrected: If one of the two classes teachers leaves the room, the children that are not of school age like the others will be required to be taken into another classroom with children of the same age. Once the second teacher returns the younger children may also return.

Each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent them from caring for children under their supervision.

The health exam for Staff 1 was 2 months overdue. The health exam for Staff 3 was 4 months overdue.
Plan of Correction: Already corrected. Will use a spreadsheet to keep track of expiring dates on employee information.
