During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

A subsidy health and safety inspection (SHSI) completed on 9/16/2025. The inspection started at 10:00am and concluded at 10:45 am. There were (4) children present and (1) caregivers; the point total was (12). The inspector reviewed (4) children?s records and (6) caregiver/household members records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-190-B
The child's information shall include the following: Proof of the child's identity and Immunization records on or before the child's first day of attendance.
Child # 1 record does not have documentation of proof of identity and was overdue by 2 years.
Child #2 record does not have documentation of proof of identity and immunization records and was overdue by 15 months.
Child #3 record does not have documentation of proof of identity and was overdue by 8 months.
Child #4 record does not have documentation of immunization records and was overdue by 8 days.
Plan of Correction: The provider will obtain the immunization and proof of birth from each child enrolled.
Standard 8VAC20-790-250-F
Caregivers who work directly with children shall annually attend at least 16 hours of training, to include the department's health and safety update course.
Caregiver #1 has not taken the department's 3 hour annual health and safety update course and is overdue by 2 years.
Plan of Correction: The provider will take the 3 hour annual health and safety update training and maintain the certificate in their record.

Repeat Violation:
The provider has not maintained a record of dates of monthly fire drills.
There was no documentation that the August 2025 fire drill was completed.
Plan of Correction: The provider will maintain monthly fire drills and record them on the fire drill record.
