Inspector Notes

An unannounced complaint investigation was conducted on 11/17/23 in response to a complaint received in the licensing office regarding allegations related to supervision, ratio, and accident reporting to families. Based on review of documentation and interviews conducted, the allegations were unable to be substantiated due to insufficient corroborating evidence. One violation was found during the investigation, but was not part of the complaint, and can be reviewed on the Violation Notice. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for the violation cited and return to me by close of business on 12/7/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Based on interview, the center did not inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurred while the child was under the supervision of the center and required outside medical attention. Evidence: Interview revealed that on October 5, 2023, at approximately 8:30am, Child A, a two-year-old, tripped and hit their forehead on a table, leaving a large lump and bruise. The center called the child?s parent at approximately 8:39am and the child was taken to the emergency room for evaluation. The center did not report the injury requiring outside medical treatment to the department.
Plan of Correction: The owner and current director was unaware that the incident was not reported properly. The staff member who was supposed to file the incident did not follow through with Licensing Standards. After talking with our inspector, the owner and current director was educated by our Inspector on how to complete the process for any future incidents.
