Inspector Notes

An unannounced complaint investigation was initiated on 11/29/23 with the director and one of the owners, based on an allegation related to the care and protection of a child in care. Documentation was reviewed, interviews were conducted, and video footage was reviewed. There was insufficient evidence to support the allegation and no violations were cited based on the allegation. One violation was found during the investigation, but was not part of the complaint, and can be viewed on the Violation Notice. An exit interview was conducted with the other owner on 12/14/23. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 12/22/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Based on review of documentation and interview, the center did not maintain a written record of children's serious and minor injuries in which entries were made the day of occurrence and included the following: 1. Date and time of injury; 2. Name of injured child; 3. Type and circumstance of the injury; 4. Staff present and treatment; 5. Date and time when parents were notified; 6. Any future action to prevent recurrence of the injury; 7. Staff and parent signatures or two staff signatures; and 8. Documentation on how parent was notified. Evidence: Interview revealed that prior to October 18, 2023, when an injury occurred at the center, staff would send a message through their app to notify parents of an injury. There was no written record kept that contained the required documentation.
Plan of Correction: The center corrected this violation on October 18, 2023. The center has an Incident Report Binder in the office to keep a paper trail of all incidents. All incidents will be done on paper as well as in procare.
