Inspector Notes

A monitoring inspection was initiated on September 25, 2023 from 9:20 am to 10:05 am and completed on September 28, 2023.There were 18 children present, ranging in ages from infancy to four years, with 4 staff supervising. There was observation of the children participating in interactive play, working on triangle activity, playing with age appropriate equipment, completing bathroom breaks. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. The center's larger playground area is currently not in use. A total of 5 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of staff records and interview, the center did not ensure to obtain documentation for all staff of a negative tuberculosis screening.

Evidence: In Staff#2's record (date of hire: 09/25/23), there was not a tuberculosis screening found for review. Interview with staff confirmed the documentation was not available for review during the inspection.

Based on observation, the center did not ensure all hazardous substances are kept in a locked place using a safe locking method that prevents access by children.

Evidence: In the Threes & Fours classroom, there were two bottles of hand sanitizer and one container of anti-bacterial hand wipes on the sink counter with the labels "keep out of reach of the children" and "warnings"
