Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 10/07/2024 and completed on 10/07/2024. The on-site inspection began at 9:55AM and ended at 10:45AM. The inspector reviewed compliance in the areas listed above. There were 4 children present and 1 caregiver; the point total was 9. The inspector reviewed 4 children?s records and the caregiver record on-site. This inspection included document review, a tour of the facility, interviews and observations. No medications were on site. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/14/2024. A POC submitted after this date will not appear on the public website.

Violations
9Poisonous cleaning chemicals were in the cabinet under the bathroom sink, accessible to children.
The Provider's current license is for children ages 1 year to 12 years. A child under 1 year old was present and enrolled.
When the Licensing Inspector asked about the cat that was previously at the facility, the Provider stated the cat was no longer living there. After seeing bowls of cat food on the kitchen floor, the Licensing Inspector again asked about the cat. The Provider stated the cat only visits on weekends, never weekdays. During the tour of the family day home, the Licensing Inspector witnessed the Provider opening a bedroom door, reaching in to engage the lock on the doorknob and closing the door so it was locked from the inside. The Licensing Inspector informed the Provider that the bedroom would need to be inspected as well. The Provider stated the door was locked and she couldn't get in. The Licensing Inspector showed the Provider how to unlock the door with her fingernail. Once the door was opened a cat was seen sleeping on the bed.
A cat was present in the home and the documentation for the rabies vaccination wasn't available for review.
There was no crib available for an enrolled infant, who is under 12 months old.
3 out of 4 children's records were missing emergency contact information, their first date of attendance, written authorization for emergency medical care, and written authorization if a caregiver is to administer prescription or non-prescription medication.
The parents of Children #1, #2, and #3 were not provided in writing the following required information: whether or not there is liability insurance in force on the family day home operation, and provisions of the family day home?s emergency preparedness and response plan.
There were no parent acknowledgements available for Child #1, Child #2 or Child #3.
3 out of 4 children's records contained copies of birth certificates without documentation verifying the date of when the original birth certificate was seen by the Provider.
