63.2 Child abuse and neglect
During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced monitoring inspection was initiated on 8/29/2025 and completed on 9/10/2025 with a local agency in response to a self-report received by the licensing office on 8/27/2025 relating to Care of Children, Emergencies and Abuse / Neglect. The inspector reviewed 4 children?s records and 2 staff records on-site on 9/2/2025. This inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to Administration and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 9/17/2025. A POC submitted after this date will not appear on the public website.
Violations
4The written discipline polices provided to parents of children enrolled in the family day home state "Corporal punishment such as spanking is prohibited". On 8/27/2025, provider #2 struck child #3 (1 year old) when she entered the day care area and discovered that child #1 had been removed from his infant seat and was lying face down on the floor.
On 8/27/2025, Staff #2 left 4 children (child #1 - 4 months old, child #2 - 2 years old, child #3 - 1 year old, child #4- 23 months old) alone in the home for approximately 8 to 10 minutes while she walked to the corner of the block where the family day home is located to retrieve children off of the school bus.
The children in the home were not supervised by an adult and no adult had an awareness of the children or was near enough to intervene when needed during this time.
While unsupervised, child #2 (2 years old) struck a 1 year old (child #3) and struck a 23 month old child (child #4). A 4 month old infant (child #1) was turned over in his infant seat where he was secured with a seatbelt, forcibly pulled out of the seat by his arm, repeatedly dragged around the room by his arm, and hit and kicked multiple times by child #2. The infant (child #1) sustained bruising and was taken for outside medical treatment to assess injuries.
On 8/27/2025, the family day home failed to protect a 4 month old infant (child #1) from other older children in care when staff #2 left 4 children (child #1 - 4 months old, child #2 - 2 years old, child #3 - 1 year old, child #4- 23 months old) unsupervised and a two year old child (child #2) pushed over an infant seat where a 4 month old infant (child #1) was buckled in, forcibly pulled child #1 out of the seat by his arm, repeatedly dragged child #1 around the room by his arm, and hit and kicked child #1 multiple times. Child #1 incurred bruising which required outside medical assessment.
On 8/27/2025, when staff #2 discovered that child #1 (4 months) had been removed from his infant seat and was lying face down on the floor, provider #2 struck child #3 (1 year old) exclaiming "Who took the baby out the seat?"
