Inspector Notes
An announced initial inspection was conducted on December 22, 2022, from approximately 12:00 PM to 1:15 PM. The center will operate from 6:00 AM to 12:00 AM and every other weekend from 8:00 AM to 4:00 PM. . The center will use 8 rooms. The boy's bathroom consists of 2 sinks and 3 toilets. The girl's bathrooms consist of 2 sinks and 3 toilets. The center has an additional bathroom with one toilet and one sink. The center plans to have transportation, the vehicle was not at the location so the Licensing Inspector was unable to inspect the vehicle(s). The center will administer medications as needed. The center is equipped with supplies, equipment, and toys. The Director and one staff member records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures

3Evidence:
1. The inspector requested the location of an in-service, nonpay telephone. There was not a working in-service, nonpay telephone during the initial inspection.
2. The provider stated that the phone service was being connected the following day.
Evidence:
1. The first aid kit did not include two triangular bandages.
2. The director stated that the two triangular bandages were not in the first aid kit.

1. There were no emergency numbers posted at the center.
