Inspector Notes
A renewal inspection was conducted on July 20, 2023 from approximately 9:10 AM to 11:30 AM. There were 16 children present in the center, ranging in ages from Infant to four years, with six staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and eight staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures

Evidence:
On the playground, the ladder to the jungle gym had a bolt missing from the top right side of the ladder. The ladder was able to move and was not secure. The jungle gym moved back and forth when children were swinging on it. One side of the jungle gym had a piece of the frame no longer attached to the jungle gym.
Evidence:
Four out of four mulch depth samples taken on the playground around the fall zones of the jungle gym and swings measured less than the required six inches.
