During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on February 24, 2025 and completed on February 24, 2025. The on-site inspection began at 8:25 AM and ended at 10:30 AM. The inspector reviewed compliance in the areas listed above. There were 12 children present and five staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on March 14, 2025. A POC submitted after this date will not appear on the public website.

The center is required to obtain a completed sworn statement prior to the employee's first day of employment.

1. The sworn statement for staff #4 was dated five days after their first day of employment.
2. The sworn statement for staff #5 was dated 20 days after their first day of employment.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe, and operable condition.

1. Boys bathroom had a broken toilet paper holder in stall #1 and stall #2.
2. Girls bathroom had one broken toilet paper holder in stall #3.
3. Infant room had peeling and chipping paint on wall with parts of the drywall exposed. Infant bathroom had broken toilet paper holder and broken door knob. When inspector tried to close the bathroom door, the door handle came off.
