Inspector Notes

A mandated monitoring inspection was conducted on November 28, 2023 from 10:15 A.M.-1:45 P.M. There were 87 children present, ranging in ages from two to six years of age, with 13 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of eight child records and 15 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of staff records, the center failed to ensure that all out-of-state criminal history background checks were completed prior to first day of employment.

Evidence:
1. Staff #2's sworn disclosure statement dated 07/23/2023 indicated they had lived out of Virginia with the past five years. Staff #2's date of employment was 08/09/2023. There was not a completed out-of-state criminal history background check in the record.
2. Administration verified that the out-of-state criminal history background check had been sent out, but they have not received the results.
Plan of Correction: Administrative staff followed up on the background check. Staff reviewed hiring procedures in a meeting on 11/30/2023. Recent new hire files were reviewed to ensure compliance.
Standard 8VAC20-780-160-A
Based on a review of staff records, the center failed to ensure that the tuberculosis screening was completed within the last 30 calendar days of employment.

Evidence:
1. Staff #1's date of employment was 08/10/2023. The tuberculosis documentation was dated 06/13/2023.
2. Administration verified the date of employment and the date on the tuberculosis documentation.
Plan of Correction: Staff reviewed hiring procedures in a meeting on 11/30/2023. Recent new hire files were reviewed to ensure compliance.
