Inspector Notes
A mandated monitoring inspection was initiated was conducted on February 7, 2024 from 11:15 A.M.-2:30 P.M. There were 46 children present, ranging in ages from three months to three years of age, with 16 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations
3Based on review of staff records, the center failed to ensure that all staff complete the Virginia Department of Education sponsored orientation course within 90 calendar days of employment.
Evidence:
1. Staff #1's date of hire was 05/27/2022. The orientation certificate in the record was dated 10/07/2022. The training was required to be completed by 08/27/2022.
2. Staff #3's date of hire was 09/25/2023. The orientation certificate in the record was dated 01/08/2024. The training was required to be completed by 12/25/2023.
3. Staff #5's date of hire was 02/07/2022. The orientation certificate in the record was dated 06/06/2022. The training was required to be completed by 05/07/2022.
4. Administration verified the dates of hire and the dates on the orientation certificates.
Evidence:
1. In Classrooms #4 there was a can of Lysol Disinfectant Spray, and a bottle labeled Bleach water in a cabinet that was not locked.
2. In Classroom #6 there was a can of Lysol Disinfectant Spray, and a bottle labeled bleach water in a cabinet that was not locked. The cabinet was unlocked with the key still in the locking mechanism.
3. In classroom #10 there was a spray bottle labeled bleach water that was not in a locked cabinet.
4. "Keep out of reach of children", and "caution" were on the front of the Lysol Disinfectant Spray.
Evidence:
In Classrom #6, on 02/07/2024, Staff #1 was observed on the carpet interacting with children. Staff #1 then proceeded to start a diaper change. Staff #1 did not wash their hands before starting the diaper change.
