Inspector Notes

An unannounced monitoring inspection was initiated on July 8, 2025 and completed on July 22, 2025 in response to a self-report received by the licensing office on June 20, 2025 relating to physical plant. This inspection included document review and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 31, 2025. A POC submitted after this date will not appear on the public website.

Cosmetics, medications, or other harmful agents shall not be stored in areas, purses, or pockets that are accessible to children.

In an infant room, staff #1 was sitting on the floor with a vape pen stored in the staff person's pocket. When the staff got up from the floor, the vape pen containing THC fell out of the staff's pocket onto the floor. The pen was accessible to infant #1 (age 12 months) who was within inches from the pen.
