The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.
The center did not request a search of the child abuse and neglect registry and the sex offender registry from one state for Staff #2, who had been employed for three weeks and was working with children at the time of the incident that occurred on 02/03/2026.

The operational responsibilities of the licensee shall include ensuring that the center?s activities, services, and facilities are maintained in compliance with these standards, the center?s own policies and procedures that are required by these standards, and the terms of the current license issued by the department.
1. The center?s discipline policy states ?our center will not practice or threaten any discipline, which is physical in nature.? The center didn?t follow its own policies when staff #2, pushed infant #1 (age 13 months) on the shoulder, causing the infant to fall on the infant?s bottom.

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.
1. On 02/03/2026, Infant #1 (age 13 months) carried a toy and approached Staff #2. Staff #1 witnessed Staff #2 use two fingers to nudge/push the shoulder of infant #1 while saying ?ewe [sic], go away,? causing the infant to lose balance and fall. Staff #2 then threw the toy across the floor.
