Inspector Notes
An unannounced unmandated monitoring inspection was conducted on 3/2/2023 from 10:40 am-11:00 am. The playground was inspected. Children were observed playing outside. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Based on observation, areas of the center, outside, were not maintained in a clean, safe, and operable condition.
Evidence:
1. There was a separation in the seam of the resilient surfacing due to a tree root, causing a tripping hazard on the playground.
2. Near the burgundy and tan playhouse, the mulch edging used to keep mulch inside the playground was split, exposing a nail, and causing a tripping hazard.
Evidence:
1. There was a separation in the seam of the resilient surfacing due to a tree root, causing a tripping hazard on the playground.

Plan of Correction: I will have landscaping come by the 13th to fix the broken border, protruding nail and tree roots on the playground.
