Inspector Notes

An unannounced renewal inspection was conducted on 8/22/2023 from 10:40 am-12:15 pm. There were 49 children (infants-5yrs.) supervised by 10 direct care staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, circle time, and playing outside. Areas of non-compliance are identified in the Violation Notice. Based on record review and interview, 1 of 5 staff files indicated that the facility did not obtain the results of a central registry check by the end of the 30th day of employment.

Evidence:
1. There was no documentation of central registry results for staff #5.
2. The start date for Staff #5 was listed as 02/21/2023.
Plan of Correction: Submit corrected central registry check on staff #5.

Based on record review and interview, 1 of 5 staff files indicated that the facility has not ensured that each staff member has submitted documentation of a negative tuberculosis screening within the last 30 calendar days of the date of employment and prior to coming into contact with children.

Evidence:
1. The TB documentation on file for staff #4 was dated 06/25/2018 and 03/28/2019.
2. The start date for Staff #1 is 07/24/2023
