During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

AMENDED
An unannounced monitoring inspection was initiated on 10/1/2025 and completed on 10/23/2025 in response to a self-report received by the licensing office on 9/29/2025 relating to behavior guidance. The inspector reviewed 1 child's record and 1 staff record on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Though not a part of the original self-report, violations that were not reported were found related to forbidden actions, hand washing, and toileting procedures, policies and procedures, nutrition, supervision of children, daily activities, and procedures for emergencies and are documented on the violation notice issued to the program.

9On Friday, September 26, 2025, at approximately 2:32 pm, video footage showed Child #1 (age 17 months) waking from a nap and beginning to cry. Staff #1, who had been lying next to the child, stood up and walked away. Child #1 continued crying for approximately three minutes before Staff #2 responded and attended to the child.
At approximately 2:35 pm, video footage showed Child #2 (age 24 months) standing in front of a classroom table when Staff #1 used their right hand to grab and lift Child #2 by the upper right arm, and then placed the child in a seat, and pushed the chair in.
At approximately 2:36 pm, video footage showed Staff #2 walking with Child #1 (age 17 months), who was crying, toward the classroom cubbies. Staff #2 offered Child #1 Goldfish crackers. Staff #1 said, ?No, don?t give them that. They do not deserve it.? Staff #1 proceeded to grab Child #1?s left hand with their right hand. Child #1 turned slightly, and Staff #1 pulled Child #1 behind them while walking between two tables. Child #1 fell on the floor and hit the back of their head. Staff #1 looked back and saw Child #1 on the floor. Using their left hand, Staff #1 pulled Child #1 up by their upper left arm. Staff #1 rubbed the back of Child #1?s head and said, ?Okay, yeah, yeah, yeah,? before handing Child #1 to Staff #3 for diapering.
At approximately 2:39 pm, video footage showed Child #3 (age 22 months) standing next to a classroom table. Staff #1 told Child #3 to sit down for snack, but Child #3 continued to stand. Staff #2 then told Child #3 to sit down. Child #3 continued to stand. Staff #2 walked over to Child #3, bent over next to them, and yelled, ?Go sit down.? Staff #2 then grabbed and pulled Child #3 by their right arm and plopped them in a chair.
At approximately 2:54 pm, video footage showed Child #4 (age 22 months) and Child #5 (age 18 months) sitting in their chairs away from their classroom table. Staff #1 walked over to Child #4 and #5 and pushed Child #4?s chair back to the table. Child #5 was sitting up on their knees, backwards in their chair, when Staff #1 used both hands to grab Child #5 underneath their underarms and plopped the child in their seat. Staff #1 then pushed Child #5?s chair in.
At approximately 2:56 pm, video footage showed that during snack time, Child #5 got out of their seat at the table and ran to the carpet area. Staff #2 saw Child #5 (age 18 months) at the carpet area, then walked over to the table, removed Child #5?s chair, and replaced it with a wooden chair that had a buckle attached. Staff #2 buckled Child #5 in the chair. Child #5 began to cry, and Staff #2 shook their head ?no? at Child #5. Child #5 stayed buckled in their chair for approximately ten minutes..
Staff #1 and Staff #2 did not ensure their care, protection, and guidance when supervising children #1,#2, #3, #4, and #5.

On Friday, September 26, 2025, video footage from Classroom #1 showed that afternoon snack time lasted approximately 30 minutes. The classroom?s daily schedule stated 15 minutes for snack time. However, the children remained seated at tables without an opportunity to engage in alternative activities after completing their snack.

SYSTEMIC DEFICIENCY
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.
On Friday, September 26, 2025, video footage from Classroom #1 showed Staff #1, Staff #2, and Staff #3 not following the center's own policies and procedures that are required by the standards regarding procedures of action in case of injury and discipline policies including acceptable and unacceptable discipline measures.
The center?s procedures for action in case of injury required teachers to use accident reports to record and communicate a child?s injury or an incident. The report included what the injured child was doing prior to getting hurt and how the teacher responded. Teachers addressed the situation with all families involved either by phone during the day or at pickup.
The center?s discipline policy defined unacceptable discipline as: roughly handling or shaking a child, restricting movement through binding or tying, enclosure in a small confined space or any space that the child could not freely exit, withholding or forcing of food or rest, etc.
Staff #1, #2, and #3 signed the center?s orientation form acknowledging the center's policies and procedures required by the standards.

On Friday, September 26, 2025, at approximately 2:34 pm in Classroom #1, video footage showed Staff #3 changing the diapers of 10 children without cleaning and disinfecting the diapering surface between each change. A paper roll was observed on the changing table; however, it was not replaced between several diaper changes.

The parents of Child #1 were not notified by the end of the day and were contacted on the next day.
