Inspector Notes

An on site announced initial inspection was conducted on December 12, 2022 from 9:50A.M..-11:15 A.M. The providers home and all areas for child care were toured. Items reviewed during the
inspection included; required posted information, first aid kid, non-medical emergency supplies, emergency preparedness, nutrition, one provider record, one household member record, and pet vaccination were reviewed. The outside play area was viewed and checked for hazards.

A recommendation will be made for an initial six month conditional Family Day Home license with a capacity of eight, ranging in ages from birth-12 years of age.
