During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on December 3, 2024 and completed on December 3, 2024. The on-site inspection began at 11:45 a.m. and ended at 1:35 p.m. The inspector reviewed compliance in the areas listed above. There were eight children present and three caregivers; the point total was eighteen. The inspector reviewed three children?s records and three caregiver/household member(s) records on-site. All other children's records were reviewed for updates. No updates needed at this time. This inspection included record review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-1
The center is required to obtain a completed sworn statement prior to the employee's first day of employment.
There is no sworn statement for staff #3, who is currently working and has been employed for three months.
Plan of Correction: Staff #3 completed the sworn statement on 12/4/24.
Standard 8VAC20-800-210-A
In addition to satisfactory completion of first aid training and CPR training, caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Staff #2 completed 13 hours of annual training.
Plan of Correction: The licensee will ensure all staff complete sixteen hours of annual training each year.
Standard 8VAC20-800-230-A
The provider shall maintain written documentation of each caregiver's applicable education and programmatic experience, applicable first aid and CPR certification, orientation, annual training, and applicable medication administration training.
The record for staff 3 did not contain the documentation of completing first aid certification. The licensee and staff 3 state the certification was completed.
Plan of Correction: A copy of the correct certificate will be obtained and added to staff 3's file.
Standard 8VAC20-800-60-B
A child's record shall contain the following information: The policy number of the child's medical insurance.
Child #1 and child #2's record did not have a medical insurance policy number.
Plan of Correction: The insurance policy number will be obtained and added to the file. In the future the caregiver will ensure all paperwork is complete.
Standard 8VAC20-800-690-D
The diapering surface shall be cleaned and sanitized after each use.
The caregiver did not clean and sanitize the diapering surface after each diapering.
Plan of Correction: The diapering surface will be cleaned and sanitized after each use.
