During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on June 9, 2025 and completed on June 9, 2025. The on-site inspection began at 10:15 a.m. and ended at 11:20 a.m. The inspector reviewed compliance in the areas listed above. There were six children present and two caregivers; the point total was 16. The inspector reviewed six children?s records and two caregiver/household member(s) records on-site. This inspection included tour of facility, record review, observation and interview. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There was no physical exam record available for Child #6, who had been in care for over three months.
Plan of Correction: The parents of child #6 will be advised to provide documentation of a physical.
Standard 8VAC20-800-460-D
The provider shall maintain documentation of the current rabies vaccination. The provider did not have documentation for cat #2.
Plan of Correction: The veterinarian will be contacted to obtain documentation of the rabies vaccination for cat #2.
