REPEAT VIOLATIONS The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. The provider did not obtain a physical for child #3 that has been at the center for over three months.
Plan of Correction: The physical will be obtained for child #3. In the future the provider will review each document to ensure all required documents are obtained.
