During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced monitoring inspection was initiated and completed on October 8, 2024 in response to a self-report received by the licensing office on October 3, 2024, relating to behavioral. The inspector reviewed 1 child?s record on-site. This inspection included document review and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to children's records and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 29, 2024. A POC submitted after this date will not appear on the public website.
Violations
4Child #1's record did not contain parent's signed acknowledgement of the receipt of the information required by 8VAC20- 800-70 and emergency contact information including: the name, office address and telephone number of the child's physician and the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. Child #1 was enrolled for one day.
On October 8, 2024, a child with special needs attended the family day home. Staffing was not appropriate and adequate to meet the specific developmental needs of the child.
Caregiver #1 did not use discipline that included techniques such as using limits that are appropriate and understandable for the child's level of development. On October 4, 2024, Caregiver #1 strapped Child #1 (age: 7 years) into a car seat and gave the child a book for less than 5 minutes.
The provider did not obtain documentation that Child #1 has been adequately immunized before the child attended the family day home. The child attended the family day home on October 8, 2024.
