Inspector Notes

An announced initial inspection was conducted today with the applicant from11:10 am to 12:50 pm. The applicant was present with an assistant and three children, ages ranging from 11 months to 2 years.There was sufficient space, supplies and furnishings for the requested age range and capacity.

Three cribs were inspected and have a manufacturer date of 7/22.

At this time, the applicant has not plans to transport or administer medication.
The family pet, Bomber is due for rabies vaccination by 10/5/23.
Four children's files were reviewed today. Please send a copy of your completed Emergency Preparedness Plan by 12/9/22. The areas of non compliance are outlined on the violation notice.

Standard 8VAC20-800-100-A
Based on a review of four children's files and an interview with the applicant, it was determined that one child did not have a physical on file within 30 days after the first day of attendance.
Evidence:
Child A enrolled on 4/25/22 and there was no physical on file.
Plan of Correction: Missing Child Physical Documentation obtained 12/1/22.
Standard 8VAC20-800-690-D
Based on interview and observation, it was determined that the diapering surface is not being cleaned and sanitized after each use.
Evidence:
The diapering surface is not being cleaned with soap and water after use nor was the diapering surface sprayed or dipped in a sanitizing solution and then allowed to air dry.
Plan of Correction: Cleaning Procedure and Approved Products for Baby Changing Station Sanitization (We have purchased a Clorox Cleaning Product that meets the EPA Registration Requirement and the 30 Second Sanitization Period, additionally, we have purchased spray bottles and mixed a Soap and Water combination to accomplish the initial cleaning of the Changing Station Surface prior to Sanitization

Based on observation and interview, it was determined that there was no refrigerator or freezer thermometer to ensure that the temperature is maintained at or below 40F in the refrigerator and at or below 0 in the freezer.
Plan of Correction: Thermometers in Refrigerator and Freezer section of appliance (The Refrigerator Section is holding an Average Temperature of 38F and the Freezer Section is Holding an Average Temperature of -2F,
