During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 12/5/24. The on-site inspection began at 9:45 am and concluded at 10:35 am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 3 caregivers; the point total was 11. The inspector reviewed 5 children?s records and 2 caregiver records on-site.
This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical prior to a child's attendance or within 30 days after the first day of attendance.

Child A enrolled 4 months ago and there is no physical on file.
Plan of Correction: I will obtain the physical.
Standard 8VAC20-800-110-B
A child's physical exam must be signed by a physician or their designee.

The physical for child B was not signed by a physician/designee. Child B enrolled over two years ago.
Plan of Correction: i will obtain the physician's signature.
Standard 8VAC20-800-60-B
A child's record, at the time of enrollment shall contain certain information to include at least two emergency contacts.
Child A , who enrolled 4 months ago, was missing one emergency contact,
Plan of Correction: I will obtain a second emergency contact.
