During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was conducted on December 19, 2024 from 12:00 p.m. to 2:15 p.m. The inspector reviewed compliance in the areas listed above. There were 61 children present and 16 staff. The inspector reviewed 5 children?s records and 6 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-500-A
Staff shall wash hands with soap and running water before and after a diaper change. Staff did not wash hands before or after changing a child?s diaper in the infant room. Children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine. Staff changed a child?s diaper in the infant room and did not wash the child's hands with soap and water.
Plan of Correction: The staff was substituting in the infant room during lunch breaks. The staff will be re- trained in the diapering procedure.
Standard 8VAC20-780-510-E
The center shall ensure that procedures for administering medication shall include methods to prevent use of outdated medication. The emergency medication for Child B expired
11/2024 and the parent had not been notified that a replacement was needed.
Plan of Correction: The emergency medication will be sent home when a new medication is provided.
Standard 8VAC20-780-510-P
When an authorization for medication expires, the facility shall notify parents that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed. Medication for Child A was onsite, but the medication authorization had expired months ago.
Plan of Correction: The medication will be sent home.
