An unannounced monitoring inspection was initiated on October 7, 2025 in response to a self-report received by the licensing office on September 30, 2025 relating to discipline. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

When time-out is used as a behavior guidance technique, it shall not exceed 1 minute for each year of the child's age. On 9/29/2025, Staff B placed a child (age 3 years) in time-out for up to 1 hour.
