An unannounced monitoring inspection was initiated on July 31, 2025 and concluded on October 7, 2025 in response to a self-report received by the licensing office on July 28, 2025 relating to discipline. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The following actions or threats thereof are forbidden: rough handling a child. On July 28, 2025, Staff 1 was observed lifting Child 1 (age 1 year) off the floor by one arm. Child 1 was seen by a physician, and was diagnosed with nursemaid's elbow.
