Inspector Notes

An announced initial inspection was conducted today from 12:00pm-1:40pm. There were 59 children enrolled with 10 staff. The physical plant, 4 staff records, 2 children?s records, medications, emergency supplies, and policies were inspected. Area is clean and safe. There was an assortment of age appropriate materials for the children in care. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Based on record review, it was determined the center did not maintain a complete record for child #2.
Evidence:
1. The enrollment form for child #2 listed a food allergy but not a current medical report to coincide with plan of action.
