Inspector Notes

A monitoring inspection was initiated on 2/23/2024 at approximately 11:30am and concluded on 2/23/2024 at approximately 1:34pm. There were a total of 53 children with 9 staff members present. Children were observed during lunch time, toy play, and engaged in group activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 6 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of staff files, it was determined that the center did not obtain an out of state criminal history name check required prior to the 1st day of employment.

Evidence: There was not out of state criminal history name check for Staff #2.
Plan of Correction: Center will obtain the out of state criminal history name check and keep on file.
