Inspector Notes
An unannounced inspection was initiated on 7/30/2025 and completed on 8/1/2025 in response to a complaint received by the licensing office on 7/29/2025 relating to allegations of special care provisions and emergencies. The inspector reviewed one staff record on-site. This inspection included, document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
