Inspector Notes

An unannounced renewal inspection was conducted on June 2, 2023, from 7:50am to approximately 9:45am. There were 19 children and 4 staff members present. The Inspector reviewed 5 children?s records and three staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during group activity, handwriting, singing songs, and free play. Medication is not administered at the center and there is no transportation provided at this time. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the owner at the exit interview.

Standard 8VAC20-780-270-A
Based on observation and interview, it was determined that the center did not ensure that the areas and equipment of the center shall be maintained in a safe condition.
Evidence:
1. A 24-inch portable fan was in use and place on top of a bookshelf. The fan was not secure and poses as a tipping and injury hazard to children in care.
2. Staff #1 confirmed that the fan was not secure and aware that it poses as a tipping hazard.
The center responded with the following: Staff will have maintenance secure the fan against the wall to prevent it from tipping or falling off the shelf.
Plan of Correction: The center responded with the following: Staff will have maintenance secure the fan against the wall to prevent it from tipping or falling off the shelf.
Standard 8VAC20-780-350-B-1
Based on observation and interview, it was determined that the center did not ensure that the ratio from the ages of birth up to 16 months is one staff member to four children.
Evidence:
1. In the infant room, there were five children to one staff member.
2. Staff #1 and Staff #2 confirmed that the infant room was out of ratio.
Plan of Correction: The center responded with the following: Two staff members that were supposed to be at the center at 8am were both stuck in traffic. The owner has informed the staff members that if there is not enough staff to stay in ratio when the children come to the center, they will ask the parents to wait until staff are present to stay within ratio of staff to children.

Based on observation and interview, it was determined that the center did not ensure that the ratio of 2 year old children and above in a classroom are one staff member to eight children.
Evidence:
1. In the preschool room, with children ranging from the age of 2 years old to 9 years old, there were 13 children to one staff member.
2. Staff #1 confirmed that the preschool room was out of ratio and aware that the ratio requirements are 8 children to one staff member.
Plan of Correction: The center responded with the following: Two staff members that were supposed to be at the center at 8am were both stuck in traffic. The owner has informed the staff members that if there is not enough staff to stay in ratio when the children come to the center, they will ask the parents to wait until staff are present to stay within ratio of staff to children.
Standard 8VAC20-780-60-A
Based on record review and interview, it was determined that the center did not ensure that each child enrolled shall contain the following information to their record: the work phone number and place of employment of each parent who has custody.
Evidence:
1. Two of the five children?s records reviewed did not have work phone numbers for both parents.
a. Child #1 (date of enrollment 04/17/2023) did not have work numbers for both parents.
b. Child #2 (date of enrollment 05/03/2023) did not have work numbers for both parents.
2. Staff #1 and Staff #2 confirmed there were no work phone numbers listed to Child #1 and Child #2?s records.
Plan of Correction: The center responded with the following: Staff will obtain the work numbers at pickup.
