During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 3/30/26. The on-site inspection began at 10:35am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were six children present and three staff. The inspector reviewed six children?s records and three staff records on-site. This inspection included document review, a tour of the facility, interviews, and observations. A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards. Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/13/26. A POC submitted after this date will not appear on the public website.

Repeat Violation
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Additional immunizations for Child #1 and Child #2 were not received every six months, which is required when a child is under the age of two years.

Standard 8VAC20-780-245-A
Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care. There was no documentation in the records of Staff #1 and Staff #2 to show that 16 hours of annual training were completed in 2025.

The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. There was no documentation to show that an annual fire inspection was completed in 2025, as required.

Shelter in place procedures shall be practiced a minimum of twice per year. Only one of the two required shelter in place drills was completed in 2025.

When centers choose to provide meals or snacks, a menu listing foods to be served for meals and snacks during the current one-week period shall list ant substituted food. On the day of the inspection, the lunch served was substituted, but not listed on the menu as a substitute.
