The areas of non compliance are outlined on the violation notice.

Violations

2
Standard 8VAC20-780-270-A
Based on observation, it was determined that the center was not maintained in a safe and operable condition.
Evidence:
The platforms and steps on the multicolored climbing and slide structure on the preschool playground was severely decaying, rusting and cracking.
Plan of Correction: We will place the play structure off limits with signage, tape and by training the teachers to make sure the structure wont be used until we can repair the platform and steps.
Standard 8VAC20-780-330-B
Based on observation and measurements, it was determined that there was inadequate resilient surface surrounding play equipment on the preschool playground.
Evidence:
The resilient surface ( poured-in-place) on the preschool playground surrounding the multicolored composite climb and slide structure was cracking, separating and had divots in the surface therefore was no longer considered an adequate resilient surface.
Plan of Correction: We will place the structure off limits with signage and tape and train teachers to make sure the play structure is not used until we are able to repair the platform, steps and poured in place.
