Inspector Notes
The center self reported a situation involving a two year old child being left unattended on their playground for a few minutes. Based on interviews conducted on 2/6/24 there was one violation involving a lack of sight and sound supervision. The violation is outlined on the violation notice.

Based on interviews, it was determined that the center failed to maintain sight and sound supervision of a child under 10 years of age.
Evidence:
On 1/26/24 at approximately 11:45 am, child #1 ( DOB 8/15/21) was left alone on the fenced playground while staff #1 and staff #2 and 9- two year old children left the playground and returned to their classroom. Two additional staff inside the center observed child #1 outside alone and returned the child to their two year old classroom. Interviews revealed that the child was left unattended between 1 to 2 minutes.
