Inspector Notes

An unannounced 60-day monitoring inspection was conducted on Tuesday, January 24, 2023, to determine the centers compliance with licensing standards. The inspection begin at approximately 10:15 AM and concluded at 11:35 AM. Today?s census included 16 children with 2 staff supervising. The center director was present for the inspection. The inspector observed children completing a worksheet. The inspector observed the inside and outside play areas. Transportation is not being provided at this time. The center's first aid kit and non-medical emergency supplies were inspected and found to be complete. The required postings were inspected and found to be in compliance. Liability insurance, health inspection, and drills were all in compliance. There were 5 children's records and 2 staff records reviewed. There were violations cited during this inspection. Based on a review of staff records, the center failed to ensure that orientation was completed no later than seven days of the date assuming job responsibilities.
Evidence:
1. Staff #1's date of hire was 11/29/2022. The orientation documentation on file was signed 1/9/2023.
2. Staff #2's date of hire was 11/29/2022. The orientation documentation on file was signed 1/23/2023.
Plan of Correction: Moving forward all staff orientations will be completed within 7 days.

Based on a review of children records, the center failed to contain documentation of an updated record for 3 children.
Evidence:
1. Child # 2, Child #3, and Child # 5?s file did not contain a current enrollment form.
Plan of Correction: All children enrollment forms shall reflect current enrollment dates of 12/7/2022.
