Inspector Notes
A monitoring inspection was conducted today, April 14, 2023, as a follow-up to the issuance of a conditional license. The inspector was on site from approximately 9:00 am until 10:40 am. The director was present and participated in the inspection. There were 11 children present, ranging in ages from three to five years, with three staff supervising. Upon the inspector?s arrival, children were finishing breakfast followed by hand washing, bathroom breaks and brushing their teeth. Circle time was observed, children reviewed colors, calendar, classroom rules and sang songs. Later children played in centers that consisted of easel panting, housekeeping writing center and tablet. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, and nutrition. A total of four child records were reviewed. Three staff records were reviewed on April 18, 2023, at the Warsaw administrative office from 11:00 am until 1:30 pm. The inspector interacted with staff and children in care.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

1. The record of staff #1, employed 11/01/2022, had an approved portability fingerprint check dated 11/22/2022, exceeding the employment date.
2. The record of staff #2, employed 11/01/2022, had an approved portability fingerprint check dated 11/29/2022, exceeding the employment date.
entity on 11/01/2022, we hired all our staff from the previous business entity.
We were under the impression that as long as staff were not working with
children we had time to obtain all the new background checks. Going forward, we have obtained all background check in the required timeframe for all new staff.
