Inspector Notes
An announced initial inspection was conducted Thursday, November 22, 2022 from approximately 2:45 pm until 3:45 pm. Policy, procedures and additional documentation was reviewed at the Warsaw office from 12:30 pm until 2:30 pm. There were no children in attendance. The inspector reviewed compliance in the areas of administration, policies and procedures, physical plant, special care, emergencies and nutrition. Compliance with some standards could not be determined since the center is not yet in operation. The center can accommodate the requested capacity of 20 children. The center will offer care for children 3 years through five years in age.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have questions regarding this inspection, please contact the licensing inspector.

A wooden fence along the designated outdoor play area has a broken gate and is missing
multiple piece exposing nails and creating sharp edges that could snap the skin of a child.
