Inspector Notes
A monitoring inspection was conducted on 04/09/2024 from approximately 10:50 a.m. to 1:25 a.m. There were 5 children present during the inspection and one caretaker present. The inspectors reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition.
Five children?s records and one provider records were reviewed. The information gathered during the inspection determined violations with applicable standards. A violation notice was issued. Please sign and complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days.
Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

The physical conducted 01/24/2023 for child, C3, age 4 years, enrolled 04/09/2024, was conducted more than 12 months prior to attendance.
Evidence:
The emergency drill documentation did not document an emergency response drill being conducted during March 2024. Caregiver, C1 reported conducting the drill but not documenting it.
Evidence:
The emergency drill documentation (fire) dated January 2024 and February 2024 did not document: the identity of the person conducting the drill, the method used for notification of the drill; the number of caregivers participating; any special conditions simulated; problems encountered, if any; and weather conditions.

The record for child, C4 did not contain an immunization record.
